Material Warranty Application Form

Issuance of the warranty is contingent upon receipt of a satisfactory flood test report and written verification of final inspection. It is recommended that this request be submitted four (4) weeks prior to completion of work so that all inspections may be coordinated and timely processing may occur.

Execution of the TITANIUM SHIELD warranty will occur only after receipt of full payment for the applicable warranty fee.
